Health Requirements
MINIMUM HEALTH REQUIREMENTS FOR SCHOOL

Healthy students perform better in school and have better attendance. They will be better prepared academically and be fit to learn if they maintain good health practices. Having up-to-date physical examinations and immunizations will help assure that our children are ready and physically able to do well in school. Children must have proof of required immunizations and health exams by October 15, 2003, or they will face exclusion from school. Please remember that a healthy child is a healthy learner.

PHYSICAL EXAMINATION REQUIREMENTS

To be received within one year of:
  • entering schools in the state of Illinois for the first time at any grade
  • level
  • entering kindergarten or 1st grade, 5th grade, or 9th grade (ages 5, 10, 15 for ungraded programs)
  • entering preschool up to age 6 (physical exam and lead screenings)
IMMUNIZATION REQUIREMENTS

Diphtheria, Pertussis (Whooping Cough), Tetanus (DTP/Td)
  • 4 or more doses
  • The last dose qualifying as a booster and received on or after the 4th birthday
  • A booster is required every 10 years
Polio Vaccine
  • 3 or more doses
  • The last dose qualifying as a booster and received on or after the 4th birthday
Measles
  • 1 dose received at 12 months or later
  • Second dose received no less than 1 month after the first dose
  • The second dose of MMR should be given between 4 and 6 years of age
Rubella
  • 1 dose received at 12 months or later
Mumps
  • 1 dose received at 12 months or later
Hepatitis B
  • Required for students entering preschool, 5th and 9th grades.
  • Catch-up is required for 6th-, 7th-, 8th-, 9th-, 10th- and 11th-graders if not received as 5th-graders in 1997
  • #1 Dose - immediately
  • #2 Dose - 1-2 months after dose #1
  • #3 Dose - 4-6 months after dose #1
Hepatitis B

Varicella (Chicken Pox)
  • Required for students entering preschool, kindergarten, and 1st grade if not received as kindergartener in 2002.
HIB (Haemophillus Influenza, Type B)
  • Required for students entering any preschool or pre-kindergarten program (not required after age 5)
  • 3 doses or more for series
  • If not received before 15 months old; only one dose required after 15 months old
